ACCOUNTABILITY FORM

If any items or equipment are broken or destroyed during the course of performing janitorial cleaning as a result of intent or negligence, I authorize JJS Commercial Cleaning, LLC to deduct the replacement or repair of those items or equipment from my paycheck(s), or I must pay out of pocket.
